”Please read a NICE guidance on vomiting and diarrhoea, gastroenteritis and I.V fluids in children, be clearer with your decision (make it relevant, MY DECISION was to start child on fluid challenge and discuss with nurses) + Be more clear with your model and background evidence.
The child comes with vomiting and diarrhoea only he has no other conditions, hickman lines, midlines, acute injuries, bleedings so please make it relevant and relate only to patients with gastroenteritis and D+V. Also a fluid challange is not a fluid bolus in our hospital. It is a calculated specific amount of fluid (based on childs weight, fluids; water, juice or water with diralyte) given every 10 mins. It also need to be recorded by parents on the chart to see if they tolerate this amount of fluids. for example child weights; 18 kg our fluid challenge formula would be:
First 10 kg – 100ml/kg/day
2nd 10kg – 50ml/kg/day
the rest kg – 20ml/kg/day
from the formula above an 18kg child would need 9.7mls of fluids every 10 mins.
