Briefly describe the data collection method(s) you are planning to use in your study (e.g., quantitative, qualitative, mixed methods).
Important: If you propose conducting a quantitative intervention, will you create your own survey or use a pre-existing survey measurement tool? If you are proposing to conduct a qualitative intervention, are you planning to conduct individual interviews (if so, how many participants) and/or focus groups (if so, how many)? If you are proposing a mixed methods intervention, what type of quantitative and qualitative research methods are you proposing to collect data?
Please describe the type of study design you are proposing to use.
Important: Please remember that we discussed several types of study designs used in Epidemiology as well as in SBS. You can use study designs from either discipline.
Briefly describe the type of intervention you are proposing and what level(s) of the Social Ecological Model your intervention will address.
Important: Your intervention must address at least one level of McLeroy’s SEM. If you are proposing to address several levels, briefly discuss each level as it relates to your intervention.
How would you evaluate your proposed intervention (e.g., evaluation methods such as process, formative, or summative evaluation)
Note: The purpose of an evaluation is to improve quality and determine your intervention or program’s effectiveness.
