Be sure to identify specific data collection strategies for each of the 5 logic model components (5ptsper column):Include 2-3 sentences that introduce your program and why the logic model is important. Then include the following for each of the 5 columns (use headings):
• Define the component – 1pt
o What is it, and what does it entail for your program.
• Describe how you will measure sub components – 2pts
o Be specific – how many times will you measure it, for how long, for who?
o Think carefully about what “success” looks like, and explain what measures will indicate success in for each sub component.
• Present your data collection overview – 2pts
o Summarize how you will obtain data for each sub component in the column.
