Behavior Intervention/Behavior Support plan (form attached) Make sure you provide a thorough rational for each of the interventions you chose and how they were chosen to match the function of the behavior. You want to be clear how the intervention will be implemented. You must describe, for example, how often attention would be provided if you chose NCR for attention. In another example you must describe exactly what extinction would look like. For example, if you were implementing extinction, exactly what would that extinction look like? You also MUST identify a data collection system for the behaviors that persons implementing the behavior plan would use. For example, what is the operational definition for each behavior and how would the persons collect the data? Further instructions are on the form.
