Social Worker:
A) Presenting Issue: Patient is a Hispanic female.
The oldest out of five children whom all share the same mother and father. Patient is dose not currently live in the household with her family as the relationship with her parents are estrange due to a recent abortion. Patient is the mother of a 4yrs old by which whom resides with her parents due to patients’ ongoing drug addiction and instability.
List at least three (3) Long Term Goal(s) for your client in order of importance and briefly justify why the goals are in this order. Utilize the SMART model of goal setting: Long Term Treatment, Family Therapy and Job Training.
Goal #1: Patient will start to look for long-term residential treatment effective 6/18/2021. Patient will be enrolled into a Long term residential treatment facility by 7/1/21. Patient will complete 3-6 months in a long-term residential facility addressing her substance abuse. By patient enrolling into a Long term residential facility this will provide patient with clean time as she has never been able to sustain due to lack of education, knowledge and support about her addiction.
Goal #2: Patient will seek a family therapist once patient has successfully completed her long-term residential treatment. Patient is to have an appointment set up with a family therapist 2 weeks after her successful completion with long term residential treatment. Patient is to see therapist at least 2x’s a month for at least 1 year. Patient, Social worker and therapist will then revisit patients progress at patients 1 year mark and discuss any further recommendations and continuum of treatment.By patient going to family therapy this will provide the patient with the therapeutic support she needs to rebuild a relationship with her family. This will also provide for possibility of other family members i.e. mother and father to join on some sessions to address/resolve any issues they had which kept them from continuing a relationship with patient. As well as rebuild a healthy relationship with patient.
Goal #3: Patient is to set up an appointment with workforce NJ to receive resume building classes as well as job training courses in a field of her interest. Patient is to set an appointment 1 week after her successfully completing her long term treatment. Patient is to complete the 6 week program of job training and 2 week course of resume building. Once patient completes resume and job training she will be provided with jobs in her field of interest to go on interviews as part of completion of the program. By patient completing this program it will give her the opportunity to go on interviews and build the confidence she needs to successfully find a stable job that can provide financially stability.
B) What resources (formal and informal) will be required for your client to meet the stated goals?
Workforce NJ, Medicaid, Treatment facility (that accepts patients without insurance due to not having Medicaid and waiting for acceptance), family therapist, AA, Office of vocational rehabilitation and Office of division and family and social services.
